Pulse Oximetry
Pulse oximetry is a noninvasive way to check someone’s oxygen saturation. Peripheral oxygen saturation (SpO2) readings are usually within 2% accuracy (within 4% accuracy in the worst 5% of cases) of the more favorable (and invasive) arterial oxygen saturation (SaO2) reading obtained from arterial blood gas analysis. However, the two are sufficiently correlated that the pulse oximetry method is useful for measuring oxygen saturation in clinical settings because it is safe, simple, noninvasive, and inexpensive.
A sensor system is mounted on a thin part of the patient’s body, typically a fingertip or earlobe, or across the foot in the case of a child, in its most general (transmissive) application mode. The blood flow rates in fingertip and earlobes are higher than in other tissues, allowing for better heat transfer.
Two wavelengths of light are passed through the body part to a photodetector by the system. It determines the absorbances due to pulsing arterial blood alone, excluding venous blood, skin, bone, muscle, fat, and (in most cases) nail polish, by measuring the changing absorbance at each of the wavelengths.
A less popular alternative to transmissive pulse oximetry is reflectance pulse oximetry. Since this approach does not involve a thin section of the person’s body, it is well suited to a universal application such as the feet, forehead, and chest, but it does have some drawbacks. Due to a lack of venous return to the heart, vasodilation and pooling of venous blood in the head may cause a combination of arterial and venous pulsations in the forehead area, resulting in erroneous SpO2 results. Patients in the Trendelenburg position or those receiving anesthesia with endotracheal intubation and mechanical ventilation may develop such complications.
History Of Pulse Oximetry
Karl Matthes (1905–1962), a German physician, invented the first two-wavelength ear O2 saturation meter with red and green filters in 1935. (later red and infrared filters). His meter was the first to detect oxygen saturation.
Glenn Allan Millikan invented the first oximeter in the 1940s. In 1949, Wood added a pressure capsule to squeeze blood out of the ear so that blood could be readmitted with an absolute O2 saturation value. The principle is similar to modern pulse oximetry, but implementation was difficult due to unstable photocells and light sources; this approach is no longer used clinically. Shaw built the first absolute reading ear oximeter in 1964, which used eight different wavelengths of light.
The first pulse oximetry was created in 1972 by Takuo Aoyagi and Michio Kishi of Nihon Kohden, a Japanese medical electronic equipment manufacturer, using the ratio of red to infrared light absorption of pulsating components at the measuring site. Ear Oximeter OLV-5100, manufactured by Nihon Kohden, was the first pulse oximeter, and Susumu Nakajima, a physician, and his colleagues were the first to test it in patients, announcing it in 1975. Nihon Kohden, on the other hand, put a stop to the advancement of pulse oximetry and didn’t apply for a simple pulse oximetry patent outside of Japan. Minolta released the first finger pulse oximeter, the OXIMET MET-1471, in 1977. Biox commercialized it in the United States in 1980.
Pulse oximetry was included in the standard of treatment for the administration of a general anesthetic in the United States by 1987. Pulse oximetry quickly spread across the hospital, first to recovery rooms and then to intensive care units, after it was introduced in the operating room. Pulse oximetry was especially useful in the neonatal unit, where patients do not survive without sufficient oxygenation, but too much oxygen and changes in oxygen concentration can cause retinopathy of prematurity, which can cause vision impairment or blindness (ROP). Obtaining an arterial blood gas from a neonatal patient is often unpleasant for the patient and is a leading cause of neonatal anemia. Pulse oximetry tracking can be severely limited by motion artifact, resulting in frequent false alarms and data loss. This is because many pulse oximeters cannot differentiate between pulsating arterial blood and flowing venous blood during motion and low peripheral perfusion, resulting in an underestimation of oxygen saturation. The vulnerability of traditional pulse oximetry technologies to motion artifact was first demonstrated in early studies of pulse oximetry output during subject motion.
Masimo implemented Signal Extraction Technology (SET) in 1995, which separated the arterial signal from the venous and other signals to calculate accurately during patient motion and low perfusion. Manufacturers of pulse oximeters have since developed new algorithms to minimize false alarms during motion, such as extending averaging times or freezing values on the computer, but they do not claim to calculate changing conditions during motion or low perfusion. As a result, there are still significant variations in pulse oximeter output under difficult conditions. Masimo also introduced perfusion index in 1995, which measures the amplitude of the peripheral plethysmograph waveform. The perfusion index has been shown to assist clinicians in predicting illness severity and early adverse respiratory outcomes in neonates, predicting low superior vena cava flow in very low birth weight babies, providing an early predictor of sympathectomy following epidural anesthesia, and improving the identification of critical congenital heart disease in newborns.
Signal extraction technology has been compared to other pulse oximetry technologies in published papers, with consistently positive results for signal extraction technology. The efficiency of signal extraction technology pulse oximetry has also been shown to aid clinicians in improving patient outcomes. In one study, retinopathy of prematurity (eye damage) in very low birth weight neonates was reduced by 58 percent at one center using signal extraction technology, but there was no reduction in retinopathy of prematurity at another center using the same protocol but non-signal extraction technology. Signal extraction technology pulse oximetry has been shown in other research to result in less arterial blood gas measurements, faster oxygen weaning time, lower sensor consumption, and shorter lengths of stay. Its measure-through motion and low perfusion capabilities also allow it to be used in previously unmonitored areas, such as the general floor, where traditional pulse oximetry has been plagued by false alarms. As proof of this, a groundbreaking study published in 2010 found that clinicians at Dartmouth-Hitchcock Medical Center were able to reduce rapid response team activations, ICU transfers, and ICU days by using signal extraction technology pulse oximetry on the general floor. A follow-up retrospective study at the same institution in 2020 found that after ten years of using pulse oximetry with signal extraction technology in conjunction with a patient surveillance system, there were zero patient deaths and no patients harmed by opioid-induced respiratory failure when continuous monitoring was in use.
Multiple clinical trials have shown that the pleth variability index (PVI) provides a new tool for automated, noninvasive assessment of a patient’s ability to respond to fluid administration, and Masimo introduced the first calculation of the PVI in 2007. Fluid concentrations that are too low (under-hydration) or too high (over-hydration) have been shown to reduce wound healing and increase the risk of infection or heart complications following surgery. PVI monitoring was recently identified as one of the suggested intra-operative fluid control techniques by the National Health Service in the United Kingdom and the French Anesthesia and Critical Care Society.
To improve the identification of critical congenital heart disease, an expert workgroup proposed newborn screening with pulse oximetry in 2011. (CCHD). The CCHD workgroup cited the findings of two massive, prospective studies with 59,876 participants that used signal extraction technology exclusively to improve CCHD detection with minimal false positives. The CCHD workgroup suggested that newborn screening be done with motion-tolerant pulse oximetry that has been validated in low-perfusion conditions. Pulse oximetry was added to the recommended standardized screening panel by the US Secretary of Health and Human Services in 2011. Prior to the evidence for signal extraction technology-based screening, fewer than 1% of newborns in the United States were screened. In the United States, The Newborn Foundation has reported nearly universal screening, and international screening is increasingly expanding.
